Altamont Flag Day E-Sports Tournament

Entry Fee: $10 for Single Players, $20 entry for Co-op

*Entry Fees will be collected the day of the tournament, or by contacting a Flag Day Committee Member to pay before.

Prizes will be awarded to top players in BOTH Single Player and Co-op Player categories!

ALL PARTICIPANTS MUST BE OF AT LEAST 10 YEARS OF AGE OR OLDER.

Reserve your spot online by filling out this form. A parent or guardian must sign for anyone under 18 years old. If you are playing Co-op list your partners name and have them fill out this form too, listing you as their partner, with their parent or guardians signature.

Agreement and Signature

By signing below, I agree to abide by all the terms and conditions set forth by the event organizers. I understand that failure to comply with event guidelines may result in being removed from the event with no refund.

Release, Indemnify and Defend

I hereby release, waive, discharge and hold harmless Altamont Flag Day Committee and all Employees past or present, City of Altamont and Council Members or Employees, any business or individual providing use of a location, building, or a lot for use for Altamont Flag Day, claims, suits, liabilities, judgements, costs and expenses for any property damage, loss or theft, personal injury or illness, death or other loss arising from participating in Altamont Flag Day.

Waiver

I hereby waive any protections afforded by any statute or law in any jurisdiction whose purpose and/or effect is to provide that a general release shall not extend to claims, material or otherwise which the person giving the release does not know or suspect to exist at the the time of executing the release. I am releasing unknown future claims.
